Secure and Accessible PHP Contact Form
I’m using this contact form on another blog (ACandleOfLove.com) I had to modify it a little bit so the fields would be named correctly. It is a lot of code if you need to edit it, but if the fields you need are there already, it works great. You can customize which of the standard fields to use in the ‘options’ section of your blog after you activate it though.

The plugin is called the ‘Secure and accessible contact form v.2.0WP’.

I am not sure if it is still being developed or not. but the last update was Nov of 2008 though, so it may be.
